OPPORTUNITY HIGHLIGHTS
Join Trinity Health At Home as a Home Care Occupational Therapist (OT) and make a meaningful impact by delivering compassionate, high-quality therapy services to clients in their homes. You’ll help individuals regain independence and improve their quality of life through personalized care plans and therapeutic interventions.
Schedule: Monday - Friday, 8am - 5pm with NO WEEKENDS OR HOLIDAYS!
Coverage Regions:
- Romulus, Bellville, Van Buren, Whittaker, Willis, and New Boston
- Dearborn, Dearborn Heights, Garden City, Livonia, Taylor, Wayne, Westland, Redford
- Oakland County: Waterford & Pontiac
- Livingston County: Brighton, Chelsea, Dexter, Howell
What You Will Do:
- Assess clients’ functional status and develop individualized treatment plans in collaboration with physicians.
- Deliver preventive, therapeutic, and rehabilitative services in accordance with the plan of care.
- Educate clients and families on safety, health promotion, and use of prosthetic/orthotic devices.
- Supervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs).
- Coordinate care with interdisciplinary teams and participate in case conferences.
- Document all care activities accurately using point-of-care systems.
Minimum Qualifications:
- Graduate of an accredited Occupational Therapy program.
- NBCOT certification and state licensure/registration.
- Minimum one year of experience or completion of a 6–8-week preceptorship.
- Valid driver’s license and reliable transportation.
- Home health experience preferred.
- Commitment to ethical standards and continuous learning.
